By signing this  you agree to the following:

  1. I am happy for my clip to be edited and used in Playlist for Life’s social media channels
  2. I understand that a full copy of the terms and conditions surrounding my consent to be featured on Playlist for Life’s social media channels will be emailed to be on submission of this form
