Log a safeguarding incident
Use this form to log an incident when you are concerned about the well-being of a vulnerable person.
Register multiple Help Points
Use this form to register a group of linked Help Points, e.g. where a head librarian wants to register all their libraries.
Register a new contact
Use this form to tell HQ about a contact you have made at a new organisation.
Register a talk / webinar
Complete this form whenever you give a talk or presentation. By that we mean, when you presented something to a group - not a meeting where you told a few people about Playlist for Life. For that, please use the 'Register a new contact' form above.
Register a helpline call
For Helpline volunteers to register a call once it has been completed.
Submit a case study
Use this form if you've come across a story that would make a great case study and want to tell our Comms team about it.

Request a resource pack
Organisations use this form when they want to request a resource pack from us but don't want to be a Help Point.
Story / image consent form
People who allow us to use their story - whether on film or in writing - must fill in this form to give us permission.
Request a speaker
Organisations and community groups can request a speaker to give a talk about Playlist for Life. We have a standard speaker's Powerpoint presentation that all staff and volunteers can use.

Other forms
There are other forms that members of the public use but that we can’t link directly to. That’s because these forms are sent out automatically and include hidden codes so we can link them back to certain events or records. But, for completeness, these include the following.
Webinar booking form
People can book onto one of our free webinars using the automatic booking system.
Webinar / training feedback form
Every participant on a webinar or training course is sent a feedback form after their training. We use their responses to evaluate our work.
Help Point materials top-up request
Help Points can request another box of printed materials from us once they have used up their last batch. This form is sent to them in the Help Point newsletter every month.
